Thank you for your interest in Reviving Wellness! In order to determine if you are a candidate for testosterone replacement we will need your lab results and your health history form. We will evaluate your information prior to your consultation.
If you are under the age of 18, attempting to conceive, or planning to conceive in the future you are not a candidate for testosterone at this time. This restriction does not apply to weight loss patients.
Reviving Wellness is a practice dedicated to treating aging men with testosterone.
Please complete the checklist before your appointment.
1. Complete the online intake form below. Once this is complete, you will be contacted by phone to set up lab work.
2. Have your blood drawn.
***Do NOT have sex or ejaculate for 72 hours prior to the blood draw (this will affect your PSA level). ***
After completing the new patient intake form above, you will be contacted by phone to set up lab work.
$150 Cash price at LabCorp—to keep things simple and typically cheaper. To receive this discount, you will need to pay RevIVing Wellness directly.
If you choose to forgo this discount and use insurance, it is up to you to find out if your insurance company will cover the cost of the labs PRIOR TO HAVING THEM DRAWN. We should receive the lab work within 1 week or less and the results will be discussed at the initial consultation.
3. Forward a copy of your most recent prostate exam to our office, if you have one.

PRICING
Male Testosterone Pellet Fees:
Initial panel of labs $150 (negotiated cash price) may or may not be covered by insurance
Initial Consultation: (60 minutes): $175
Male pellet insertion (every 5-6 months): Approximately $800 per pellet visit
Nutraceuticals (Thyroid, DHEA, HRT complete T, ADK, etc)- should any of these supplements be recommended/added to your treatment plan cost could be an additional $35-200/month
Follow up labs post initial pellet insertion at 4 weeks: $50 (negotiated cash price) may or may not be covered by insurance
Follow up visit post initial pellet insertion at 5 weeks to review post pellet lab work. No charge (included in initial consult fee)
Yearly lab panel thereafter $150 (negotiated cash price) may or may not be covered by insurance
The average male hormone TRT patient will spend approximately $250 per month.
Male Testosterone Shot/cream fees:
Initial panel of labs $150 (negotiated cash price) may or may not be covered by insurance
Initial Consultation: (60 minutes): $175
Testosterone shots $175 monthly subscription (typically weekly injections or divided dosing 2x/week)
Testosterone cream $150 monthly subscription (typically twice a day application to testicle)
Follow up labs 6 weeks after starting shots: included
Follow up appts every 3-6 months: included
Yearly lab panel thereafter $150 (negotiated cash price) may or may not be covered by insurance
Nutraceuticals (Thyroid, DHEA, HRT complete T, ADK, etc)- should any of these supplements be recommended/added to your treatment plan cost could be an additional $45-150/month
WEIGHT LOSS
If you are interested in both testosterone replacement and weight loss they both will be covered in the same consultation.
Also, if you refer your wife/partner, we will give them a 10% discount on their 1st pellet visit!
