After completing the new patient intake form above, you will be contacted by phone to set up lab work.
$150 Cash price at LabCorp—to keep things simple and typically cheaper. To receive this discount, you will need to pay RevIVing Wellness directly.
If you choose to forgo this discount and use insurance, it is up to you to find out if your insurance company will cover the cost of the labs PRIOR TO HAVING THEM DRAWN. We should receive the lab work within 1 week or less and the results will be discussed at the initial consultation.

Female Hormone Pellet Fees:
Initial panel of labs $150 (negotiated cash price) may or may not be covered by insurance
Initial Consultation: (60 minutes): $175
Female pellet insertion (every 4 months): Approximately $400 per pellet procedure visit
Nutraceuticals (Thyroid, HRT complete T, DIM, etc)- should any of these supplements be recommended/added to your treatment plan cost could be $35-200/month
Follow up labs post initial pellet insertion at 4 weeks: $50 (negotiated cash price) may or may not be covered by insurance
Follow up visit post initial pellet insertion at 5 weeks to review post pellet lab work. No charge (included in initial consult fee)
Annual lab panel thereafter $150 (negotiated cash price) may or may not be covered by insurance
The average female hormone pellet patient will spend approximately $225-250/month.
